What is knee replacement surgery?

Knee replacement surgery or Knee Arthroplasty is the surgical procedure that replaces an injured knee partly or completely using metal implants. It is mainly done to relieve pain or to acquire back the motion of the joint lost due to the injured knee. It is usually done for elderly patients with severe arthritis (osteoarthritis, rheumatoid, psoriatic arthritis ) and younger patients with other forms of a knee injury.

The knee joint is the tibiofemoral joint, with the proximal tibia and distal femur joined together along with the kneecap (patella). The whole structure lies inside a synovial capsule filled with synovial fluid which helps in shock absorption. Ligaments, muscles and other structures help to keep the whole unit strong and stable.

The cartilage between the joints wears out (osteoarthritis), mostly after the age of 50. The lack of cartilage between the bones leads to the pain of high intensity and even reduces the range of motion. People undergo surgery to get pain relief and regain the motion of the joint.

Types of Knee Replacement Surgery?

Several types of Knee Arthroplasty are performed in India. The surgeon decides which type to be performed according to the condition of the knees.

The main types are :

  1. Total Knee Replacement

The most commonly done surgery in which the damaged bone and cartilage is removed totally from the joint and replaced with an implant. The implant is made up of metal, plastic or maybe some ceramic.

  2. Partial Knee Replacement

As the name suggests, the surgeon replaces only a part of the knee joint with an implant. This is done when there is just a part of the joint is damaged.

  3. Revision or Complex Knee Replacement

Usually done for patients with bone loss, weak ligaments or deformity and for patients undergoing their second or third knee replacement. This is a much more complicated process. The implants have a longer stem which can be encased in the bone cavity.

  4. Bilateral Knee Replacement

This type is needed when the patient has pain or arthritis in both knees, and both knees are replaced simultaneously. This is much cheaper than staged replacement as this gets over in one go.

  5. Minimal Invasive Surgery

It is a knee replacement surgery with a smaller incision. Surgery is done via a small incision (nowadays as small as 3-5inch deep) which is less painful and the recovery time is much lower. It is also pain-free for the quadriceps and other muscles in front of the thigh.

Who needs knee replacement surgery?

  1. Elderly patients suffering from arthritis (rheumatoid arthritis, osteoarthritis) whose cartilage between the bones has been worn off.
  2. Younger patients with joint injuries usually undergo this surgery to get relief from pain and regain the full motion of the joint.

The time comes up for surgery when the pain affects the day to day activities adversely such as walking, climbing stairs and even sleeping.

Pre-surgery preparations:

You need to get admitted 1 day prior to the surgery. Before the surgery, you need to make sure you have all the required tests done. This could be

  • A physical examination,
  • Lab tests
  • Radiographic examination

You could also discuss with your doctor about the choice of implants according to your budget.

The advantages and disadvantages of different implants will help you choose which implant is the best for you.

Post-surgery care:

After the surgery, you would need to stay in the hospital for a few days, which varies according to your condition and chance of developing an infection. Usually, you will be under hospital observation for 3-5 days or at most a week.

In addition, the government of India supports medical travel and have formed norms for making it safe for travelers. So in all aspects, India is a go-to country if you want your knees replaced.

How long does it take for knee replacement surgery?

The surgery takes about 2-3 hours. The patient should spend maybe two or three nights in the hospital under observation. After that, patients should move to a rehabilitation center for a few weeks depending on the condition and level of recovery. The recovery depends on proper rest and physiotherapy the patient gets.

How much time will I take to walk post-surgery?

At first, you would need a walker for the first few weeks, then slowly you can move on to crutches. With proper physiotherapy and rest, a normal person should start walking on his own in 4-8 weeks. Your knees continue to improve over this time and become its best at about 8-12 months.

How painful is knee replacement surgery?

The surgery alone is painless as you will be given anesthesia. But you will experience some pain immediately after the surgery which will diminish within 4-5 days. After that even if pain persists, it would be mild, and the doctor will prescribe pain killers for pain relief.

How is life after knee replacement surgery?

Life post-surgery is very good as there is a better quality of life with the loss of pain. The patient will regain the movement of the joint to a large extent within six months. Life will become normal from anywhere between 6 months-1 year for most patients.

What activities can you do post-surgery?

You can do normal activities such as walking within a few days. Mild exercises are doable after your rehabilitation, say 6 – 12 weeks.

Extreme exercises such as running, jumping, cycling uphill should be avoided until your knees get in proper condition. Usually, you can do all sorts of activity within 6 months-1 year but varies from person to person.
