What is Gynecomastia?

Gynecomastia, also known as man boobs, is a very common condition in men in which the breast tissue enlarges. It happens mainly due to the disorder of the hormones- testosterone, and estrogen. The reduction in androgens (testosterone) or increase in estrogen (female hormone) in men leads to an increase in the size of the breast tissue.

Everybody has breast tissue in their bodies. In most males at the time of puberty, the breast tissue gets suppressed. But the hormonal changes in some leads to the growth of the breast tissue. It mostly happens during puberty. But in some males, it can also happen later in life due to a change in lifestyle and other reasons.

How do you know whether you have gynecomastia?

Obese people usually mistake fat tissue for gynecomastia. In obese or fat people there can be fat accumulation in the chest region. This can be distinguished from gynecomastia if you examine it carefully. Gynecomastia would be more like two kinds of tissue that have different hardness and structure. The breast tissue would be harder and more prominent around the nipple.

Treatment of Gynecomastia

Gynecomastia usually doesn’t need any treatment as it is not life-threatening. But you can always treat it if it affects your confidence. 

You can either use medications or go for surgery depending on the condition.

Medication

In some men, gynecomastia can be treated using medications for breast cancer such as:

  • Tamoxifen (Soltamox)
  • Aromatase inhibitors, such as anastrozole (Arimidex)

But it is dangerous and not advised to take medication on your own. Always consult a doctor before taking any medication.

If that doesn’t help you can always opt for surgery.

Surgery

Pre-surgery

  • Two weeks before surgery, stop all aspirin-containing products, medications that contain blood thinners (Ibuprofen Motrin, Advil, etc.), vitamin E and herbal supplements
  • Quit smoking and nicotine use at least four weeks before surgery. Nicotine can impair and delay healing.
  • Perform all the lab tests and radiographic tests told by the doctor prior to the surgery.
  • The doctor decide which type of procedure is right for the condition.

During the surgery

  • Initially, anesthesia is given
  • The team of doctors does the surgery in either one of the following methods.

Liposuction

This type of technique is used when gynecomastia is the main reason for the fatty tissue present. The doctor inserts a small cannula, a thin hollow tube through several small incisions made on the skin. The cannula is moved in a controlled motion to loosen the fat tissues. The loosened fat tissue is removed using vacuum suction. 

Mastectomy (Excision)

This is the surgical procedure done mostly with several small incisions to remove the glandular breast tissue. The most common incision is intra-areolar incision, or Webster incision, which extends along the circumference of the bottom half of the areola in the pigmented portion.  It is done when fat tissue is less present.

Combination technique

This the combination of mastectomy and liposuction when both fatty tissue and glandular tissue are prominent. The doctor decides how to carry out the procedure depending on your condition.

Post-surgical care

  • You might have to stay in the hospital for maybe 2 days to keep in check.
  • Keep some comfortable shirts for wearing for a few weeks after the surgery.
  • You should be taking rest at home for 8-20 days. 
  • Avoid vigorous movements of the shoulders.
  • Avoid everything that can cause an infection to the area of the incision.

FAQ

Am I too young for surgery?

Usually pubertal gynecomastia resolves within 3-4 years if it will be resolved by itself. So it is better to wait till the end of puberty to opt for a surgery.

Will gynecomastia related to drugs or medication go away if they stop using them?

It varies from person to person. Some people have said about improvement after quitting drugs. But some found no results.

Can you treat gynecomastia by diet and exercise?

No, breast enlargement due to gynecomastia would not go away fully on diet and exercise. The fat tissue may go away but the enlarged breast tissue remains as it is. But if the breast enlargement is due to obesity or not due to hormonal imbalances, it is called pseudo gynecomastia. The enlargement is due to only fat. This would go away with diet and exercise.
