Hydronephrosis

Hydronephrosis isn’t a disease rather a condition or disorder. The condition refers to the swelling of one or both the kidneys due to the accumulation of urine. Urine fails to drain from the kidney to the bladder. Mostly, happens in only one kidney but rarely happens in both. 

In normal circumstances, the urine flows from the kidney to the urinary bladder through the two ureters. Then it gets expelled through the urethra, But in this condition, a blockage in the ureters due to kidney stones, blood clots or scarring. The block cause urine to accumulate in the kidney and leads to enlargement, It is a secondary disease that occurs due to any other underlying disease. It is a structural disorder and happens due to a blockage in the urinary tract. It happens 1/100 of newborn babies.

If not treated for a long time, it can lead to permanent kidney failure, pyelonephritis, and sepsis.

Causes

  1. Vesicourethral reflex. It is a condition where the muscle at the junction between the ureter and bladder doesn’t work. This forces the urine to flow back into the kidney. 
  2. Any blockages in the ureters, which can be due to:
  • Kidney stones
  • Enlarged prostate
  • Certain cancer affecting the urinary system
  • Incorrect development of the urinary system at birth
  • Blood clots
  • Scarring
  • Pregnancy. The increased size of the womb can cause blockage of the ureters.

Treatment of Hydronephrosis

Medication

Usually, the treatment is done for the underlying cause, such as kidney stones or blood clots. Most of this can be carried out without surgery. In the case of infections, antibiotics can be used to treat the condition. In some severe blockage cases, the excess urine should be removed using a catheter or a special tube called nephrostomy. 

Surgery

Pyeloplasty

Only severe cases need surgery and is called pyeloplasty. It is done to remove any sort of ureteropelvic junction obstruction (UPJ). 

The procedure is done under general anesthesia. The heart rate and pulse are constantly monitored. A 2-3 inch cut is made below the ribs to gain access to the urinary system. The procedure is done by removing the blocked part of the ureter and connecting the healthy part directly to the kidney. The residual urine is drained out using a catheter or a stent is placed. The stent drains out the excess urine and is removed after healing.

Minimally invasive pyeloplasty

A small cut of 8-10 mm is made below the ribs and a robotic tool is introduced. The tool has a camera attached and the surgeon carries out the surgery. The rest of the procedure is almost the same, the blocked ureter is removed. The healthy part of the ureter is connected directly to the bladder. A urinary stent might be placed.

Post-surgery

  • The patient remains hospitalized for 2-3 days. The diet is confined to ice chips and liquid food during this time.
  • You might experience some pain a few days after the surgery due to swollen ureters. It is completely normal. Painkillers will be prescribed. But don’t hesitate to consult a doctor if you experience severe pain. 
  • The kidney drain might not be good at first but it improves with time.
  • The stent would be removed a few weeks after the surgery and ultrasound are done to check your kidneys. 
  • Small walks are advised. Being in bed for a long time is not recommended. But avoid heavy lifting for 2-3 weeks. You can usually return to normal activities in 3-4 weeks.
