What is spinal fusion surgery?

Spinal fusion is a surgical procedure in which two or more vertebrae are permanently joined into one bone without space in between. In certain cases, an extra bone is used to fill the space that exists between two separate vertebrae. When the bone heals, there’s no longer space between them. The procedure reduces flexibility but relieves pain caused due to movement. The idea is that immobilizing the injured vertebrae stops the pain.

It can be done in a variety of ways:

  • Adding bone graft to a segment of the vertebral column.
  • Set up a biological response that causes the bone graft to grow between the two vertebral elements to create a bone fusion
  • The boney fusion – which leads to one fixed immovable vertebrae. It is done by fusing two vertebrae together.

Types of spinal fusion surgery

It can be classified based on the position of the spine at which the surgery is done:

Lumbar fusion

The surgery is done in the lumbar region usually for degenerative changes of the discs. The degenerated discs can come in contact with the nerves and causes pain.

Cervical fusion

This is done to the first few vertebrae, i.e, first 7 vertebrae.

Thoracic fusion

It is done to the thoracic vertebrae, named T1-T12, they are twelve in number. Mostly done due to trauma and deformities.

  • Deformities of the spine. Spinal fusion can help correct spinal deformities, such as a sideways curvature of the spine (scoliosis).

Pre-surgery

  • The doctor would ask you to take x-rays, CT scan or MRI scans to evaluate your condition and decide how to perform the surgery. Blood tests are also required pre-surgery.
  • Over-night fasting will be needed and medications will be controlled.
  • The hair at the region of incision is removed, if there is any.
  • Your heart rate and O2 levels are measured continuously.

During surgery

Incision

The incision is made to reach the vertebrae and perform the surgery. It can be anywhere on the neck, or the back, or on either side of the spine depending on the type of surgery you require. For cervical fusion, the incision is made on the back of the neck. For thoracic fusion, the incision is made either on the back on top of the spine directly or on either side of the spine. 

Bone graft

The bone graft required for the surgery is usually taken from your body, mostly from the pelvis. The surgeon makes an incision on the pelvis region, take the bone graft, and closes the incision. The bone graft is placed at the site for fusion.

Fusion 

The bone graft is placed between the two injured vertebrae. The vertebrae and the graft is kept in place until it fuses. It is done using metals, screws, bolts, and rods. Special rods are used in Scoliosis treatment, The implant used totally depends on why you are performing the surgery and your condition.

Post-surgery care

  • You would probably find yourself in the observation room while your anesthesia wears off.
  • A hospital stay of 3-4 days is needed to prevent infections and other complications.
  • You may experience pain and discomfort but your doctor will prescribe medications for it. 
  • Even after going home, be sure to check for infections. The signs include wound drainage, shaking chills, swelling or tenderness, fever higher than 100.4⁰F. 
  • Physical therapy should be started as your doctor instructs. 
  • You will have to wear a brace for a while to keep the vertebrae unmoved.
  • It would take some while until it fuses and heals. So avoid lifting heavy weights until then.
